Dementia Care Link Worker – West Mendip area

The Dementia Care Social Prescribing Link Worker can support patients that are registered with a West Mendip GP practice, such as Wells Health Centre, Wells City Practice, Glastonbury Health Centre, Glastonbury Surgery and Vine Surgery. They are part of the GP practice team.

They are able to provide support to Dementia patients and their families/carers helping to maintain their independence and increase their sense of wellbeing.
