Health Connections Manager - West Mendip
There are lots of ways you can receive support and find out about local opportunities to benefit your health and wellbeing. The Health Connections social prescribing and health coaching team are based within a health and wellbeing hub that supports all 5 local GP practices in West Mendip. They work closely with a team of Care Navigators, Care Co-ordinators, clinicians including a GP, clinical pharmacists, occupational therapists and a hospital discharge reviewer.
Health Connectors can support anyone aged 18-100+ to connect with local groups and projects, improve emotional wellbeing, make lifestyle changes, access carers’ support, secure financial assistance, find support for addictions, access digital help and more. They also work in partnership with health, social care and community professionals to assist people who are living with complex health and social issues.
If you’d like to learn more, you could start by searching our online directory which lists a wide range of groups, projects and services in your area. You could also join one of our regular Talking Cafes in Street, Wells or Glastonbury. Many people benefit from attending our pain coaching groups and Habits 4 Health programme, which we run regularly in different locations and which covers sleep, eating well, movement, emotional wellbeing and tiny habits. Our aim is to support you in making sustainable changes that matter to you.
A Health Connector could offer you a series of one-to-one appointments, either in person or over the phone depending on your preference. You might also be interested in training as a Community Connector – enabling you to signpost others in your local area to support. We can help you to identify and take the right steps forward.

Pain coaching groups
We run regular groups in West Mendip to support people with a range of health conditions, who experience persistent pain. The groups incorporate coaching and facilitate peer connections. Group sessions feature content developed by 10 Footsteps to Living Well with Pain
Pain Cafe
We run a monthly Pain Cafe in Glastonbury on the first Monday of the month at the St John’s Church Conference Hub, BA6 9DR. This is a space where people are able to share tips and experiences about living with pain. Guest speakers attend, including different complementary therapists.

Our Health Connectors lead a gentle Health Walk in the centre of Glastonbury every 2 weeks.
Health Walks are regular walks in local communities, with the aim of supporting health and general wellbeing, including improved mood, social connection, better sleep and weight management.
The walks range from gentle, to more moderate and advanced – and also take place in Street, Wells and across other parts of Mendip.
The Health Walk programme in Somerset is hosted by Somerset Activity & Sports Partnership and details of all walks in the local area are listed on their website.
