Life After Stroke Group
Somerset Partnership NHS Trust now offer a ‘Life after stroke’ group to patients in the Mendip Area. The group is a 10 week programme that offers: An initial assessment of an individual’s needs. A weekly group-based exercise programme. Advice and opportunity for peer patient and carer support in the form of weekly discussion topics on a variety of subjects. These range from advice on prevention of a further stroke to how to get the most out of life after a stroke. Information on services available from other organisations.Referral for appropriate Mendip patients is usually automatic if a patient has suffered a new stroke and goes through the Somerset Partnership Stroke Pathway. However, previous patients whom may benefit and have not had this opportunity in the past can be referred via a letter from their GP / other Health Professional.
