Our dedicated team of Caseworkers and Technicians will provide advice and practical help before you go home, or within the first few weeks after you are home such as:
Free support –
Help you set up food delivery services
Meet and Greet you at Home following your stay in hospital
Provide benefit and welfare advice
Help you sort out utility bills and other post whilst you have been in hospital
Carry out a Home Safety check to ensure you are safe and secure
Carry out a Home Energy Assessment to keep you warm, well and reduce fuel poverty.
Make arrangement for urgent shopping requests for those clients who have no one else to help.
Telecare Life Line alarm for 8 weeks for those discharged from hospital. After the 8 weeks there is a payment for the alarm.
Support where there is a charge –
Make arrangements for the fitting of key safes, home improvements and adaptations.
Moving of beds or other furniture within the home.
Arrange the installation of Telecare Life Line equipment which includes an 8 week free trial then it becomes a chargeable service.
